1. Professional Licensing & Complaints
The Texas Behavioral Health Executive Council (BHEC) investigates and prosecutes professional misconduct committed by Texas licensed professional counselors, marriage and family therapists, psychologists, and social workers.
If you have a complaint about a professional service received from Mosaic Counseling Centers of East Texas, you may contact the Council to file a formal complaint using the information below:

2. Protected Health Information & Medical Records Requests
In accordance with the Texas Health & Safety Code, Section 181.105, you have the right to request, inspect, and obtain a copy of your protected health information and medical records.
  • How to Request Records: To request a copy of your records, you must submit or deliver a request in writing to Mosaic Counseling Centers of East Texas, Client Services Department to 6141 Paluxy Drive, Tyler, Texas 75703, Attention: Client Services.
  • Processing Time: Upon receiving your written request, we will provide access to or copies of your records within fifteen (15) business days.
  • Fees: A reasonable, cost-based fee may be charged for copying, mailing, or preparing a summary of the records, as permitted under Texas law.
3. Privacy Complaints
If you believe your privacy rights regarding your health information have been violated under the Texas Medical Records Privacy Act or HIPAA, you have the right to file a complaint.
